This is a listing of students who are missing the required Medical and Rules forms for choir. Students with incomplete paperwork will lose points from their six weeks average.
We are making every effort to help the students complete the now past due assignment. We have posted lists in class, verbally reminded them, and now posting notices on our website. I cannot do anything more to prevent the loss of points from your grade.
We have checked the forms twice for accuracy. We think this is an accurate list of missing documents. If you believe that you have completed the form, please come by the office on Monday and show it to me in the book. There were a few forms that were unreadable. A signature of “squiggle-squiggle-line” prevented us from giving proper credit.
If you would like to either confirm your documents or submit the required paperwork, we will accept these documents until 12 noon on Monday. After that you will lose the points from your average. Additional blank forms continue to be available in the choir room.
If your name is not on this list, we have all of your paperwork on file and complete.
